What is the offer?
The CSSC East Yorkshire Area Association wants to play its part in helping its members maintain a healthy lifestyle and so are offering a 10% subsidy to any local member who takes out annual membership at a gym of their choice.
Please note that this offer is subject to a maximum payment of £50 in any calendar year and applies to one gym membership only.
How do I claim?
Fill in the claim form, attach the receipt for your membership and post it to;
David Moore, HMRC, Cherry Court, Ferensway, Hull, HU2 8AQ.
Please mark the envelope “Staff - Restricted”
What happens next?
All claims will be checked to ensure that they meet the criteria.
