York Area CSSC Christmas Meal Offer 
APPLICATION PROCESS
(Applications must be received by 10/1/2012 at the latest)
1.Complete the application form including the details of each CSSC member taking part and their signatures. If a member does not know their CSSC number, they should telephone CSSC Head Office on 01494-888444 to obtain it.
2. Send a hard copy of the form with the receipt attached to the address on the bottom of the form.
3. We will then pay a cheque to the organiser for the total amount of rebate (e.g. 8 members attending, £7 rebate agreed per member = cheque for £56 paid to the event organiser).
4. A random check of 10% of those receiving a rebate will be undertaken to ensure that they did in fact take part in the event and in return received their £7.
5. Any discrepancy in the 10% check will lead to a 100% check being undertaken. If a subsidy is claimed for a member found not to have taken part in the event, the subsidy must be repaid.
6. Meal must cost £12 (excluding drinks) or more per person to qualify.
7. Each CSSC member can only claim for one meal.
